It means to be confident in your actions and aware of what's going on around you in the world. For instance, calling yourself "Queen of Dumb Questions" is the opposite of sophisticated.

Sopthisticated means to be aware of and able to interpret complex issues.

It means to put on the aires.. to be haughty and a better than thou in actions

My guess is that it means high maintenance. One would tend to make things as Webster defines it, "complicated or complex".
