It means that you share your heartbreaks with people whether they care or not. Its not bad really, it just means that you are emotional and you dont contain your feelings. Your heart on your sleeve means that it is out there for everyone to see. You show your feelings by the way you talk or act or both. You sound really kind to me. I bet you are.

It means that you leave your heart open and honestly for the world to see. If it gets hurt in the process, that is part of the process. Otherwise, "SHOUT IT OUT" and move on. Life is about taking chances and learning from that. It makes us stronger.
