If the narrow piece is naturally-occurring, it's usually called an isthmus. One of these connects New Brunswick, Canada to Nova Scotia, Canada (Isthmus of Chignecto).

If the narrow piece is at least partially man-made, it's called a causeway. One of these connects mainland Nova Scotia with Cape Breton Island (The Giant's Causeway).
