Are condoms always affective for birth control and what are their chances for not being affective??

Yes they are mostly. But they can breaks or have holes.
After ejaculation, before the erection is gone, the condom must be removed from the vagina. Grasp the condom at the base of the penis and hold it as the penis is withdrawn.

If you don't put it on right or if you use one too small it wont work well at all

No, they are not always efective. They can rupture or just have small holes in them which sperm can travel through. It is always better to have a back up method handy, like BC pills, or foam.
