I received a M.D. degree at China Medical University in Taiwan, and a MPH from Tulane University in Louisiana. I also entered the Summer Session of Public Health of Harvard University on 2005.
In 1994, I worked in the Genuine-Bissau of West Africa for one year.
After ten more years, I had kept the nice family and money. (I don't know what that means, but I think you want to say this: "For ten years, I lived comfortably with my family and financially.") However I would like to work in Africa again now.
So I am applying for the DPH of Population and International Health and focus on Population and Reproductive Health, and I intend to concentrate in the epidemiology of infectious diseases.
By way of the school of Public Health, I hope to intensify the knowledge of public health. Then I could go to Africa.
If you can’t accept me for the DPH program, I would like to change to the no-degree (1year) population and international health course. (This is okay, but you could say it better like this: "In lieu of being accepted into the DPH program, I request to take the no-degree (1 year) population and international health course."

The whole thing needs a re-write. I must say that if your English is so poor it would be a worry whether you could understand enough to work in the medical field where English is spoken. You might not make yourself understood to a patient, or you may misunderstand an important instruction.
