A rectal exam means a examination of the rectum. This usually occurs when there is a certain injury in the anus or the rectum. Due to an improper diet or a lack of physical activity, your anus might be slightly injured during bladder.
Try increasing the amount of fiber in your diets and make sure you drink as much water as you can. You may also take fiber supplements.
There is a medication you can take, should you have serious constipation. THis medication is called "Colace".Ask your doctor about it.

The digital rectal examination (DRE, Latin palpatio per anum or PPA) is the simplest procedure. The patient is placed in a position where the anus is accessible and relaxed (lying on the side, squatting on the examination table, bent over the examination table, etc). The physician inserts a gloved and lubricated finger into the rectum through the anus and palpates the insides.
It's not exactly painful, but rather embarrassing.

Your rectum is your anus (where the poop comes out). A rectal exam is where they examine your rectum.
