The agent of 'death' is a bit misunderstood because death is, in and of itself, not a 'state'. Death is the nanosecond moment where a person exchanges one set of ideas about themselves for another.
Death, therefore, is more of an escorter, a 'help desk' assistant whom one turns to in order to be guided to where one needs to go. As such, there are any number of people or entities who can perform the job quite adequately; some are even person specific. Imagine Death as a job, like being a cook. The spectrum can range, from being quite specifically educated (pastry chef) very broadly educated or even very vaguely educated (short order cook). There is no such a thing as 'THE' cook, per se, because it is based upon ability.
Now, if you are talking about linneage (who was the first cook, for example) as being the determing factor, then God (or whatever creator you imagine) was the first Grim Reaper.
Personally, I like the way they handled the idea of Death in the video game Grim Fandango. I wouldn't say I got my idea from there, but I will say they 'realized' my idea in ways I would never have thought of.

I'd rather meet the Grim Reaper head on like Bruce Lee in Enter the Dragon than two step with some cloud boy like Brad Pitt in Meet Joe Black.
At least with the Reaper - If I did die it would be at the hands of a sycle and not by my own hand slicing my wrists watching some chick flick.

no. that is one thing that definately isn't real. It started of hundreds (maybe thousands) of years ago as a myth/legend and over the years it has changed and it just isn't real, well the grim reaper everyone knows about now certainly isn't real but maybe that myth/legend is.

i dont think hes real, its just a theory that people give as an answer to the question "where do you go after you die?" : the grim reaper uses his long axe to rip your heart and then takes you with him to the underworld.
i mean why do ppl believe this crap?

I believe that 'Death' is a spiritual entity that helps guide people's souls from this world to the next when they die, and that it has been given many faces by people over time.

The figure is, merely, a way to personify death.
