Sirius Black died in the fifth book, Harry Potter and the Order of the Phoenix.
He died while fighting Death Eaters (Voldemorts followers) in the Ministry of Magic's Department of Mysteries. He was killed by his evil cousin, Bellatrix Lestrange, when she hit him with a curse and Sirius fell backward into a veil in the middle of the atrium.
Little is known about this mysterious veil, but many fans suspect it to be where wizards go when they die. This is because Harry can hear voices behind it, and suspects that they are of his dead parents.

Huge spoiler warning for anyone who hasn't read Phoenix or any Potter book at all.
Oi. Sirius Black did not die in Prisoner of Azkaban. That is the book he is first introduced and Harry learns who he truely is.
He dies in Harry Potter and the Order of the Phoenix. Book 5.
Harry had been tricked into going to the Department of Mysteries by Voldemort. He had been sharing visions with Voldy throughout the entire year and after witnessing the attack on Mr. Weasley, he percieves the vision of Sirius captured in the Ministry of Magic to be true. He flys to his godfather's aid along with Ron, Hermione, Neville, Luna and Ginny. When they arrive they discover the prophecy, which is what Voldy was after all along.
After picking up the prophecy, Harry and the other's are ambushed by a group of Death Eaters. They are determined to get the prophecy for their Master. Harry refuses and a battle ensues. He realizes at that moment that his hero's heart lead his friends into a death trap.
Luckily, they are saved by the Order. Sirius included. Dueling begins. Sirius begins battling his cousin, Bellatrix and she hits him with a particulary nasty spell that sends him spilling into the Veil. There is nothing noone can do to save him. He is gone. Through the archway.
Noone knows exactly what the Veil is, they just know that noone returns from it. Or it's what we've been told.

Then people from the Order come to save him. That's how Sirus Black dies.

Theoretically 5th book.
He was dueling with Bellatrix, and was pushed behind a curtain hanging on an arch.
I say theoretically, because nobody in fact had seen the body. Harry heard some voices from the arch, whispers more like, which drew him towards the arch, but no one said that going through the arch would mean death. In fact no one ever said what was that arch. All we know is that Sirius felt through and didn't go out, even when Harry started calling him as if someone was ripping his skin off of him... .

He died in the fifth book - the Order of the Phoenix - by falling past a veil and into an archway after a duel with his cousin, Bellatrix. He seemingly just dissapeared. Although there is still mystery surrounding this death, Harry has come to accept it, and hasn't made much effort to figure out anything about how Sirius died "just beyond the veil."

Sirius died in the fifth book, Harry Potter and the Order of the Phoenix, because a stunning spell hit his heart, therefore stunning his heart so it could no longer beat. After this spell hit him, he went behind a veil to a room Harry Potter had dreams about before.

Sirius Black, Harry Potter´s Godfather dies in the fifth book, The Order of The Phoenix, when almost at the end, he is cursed by Bellatrix Lestrange, in the Ministry of Magic, when they all are fighting against Voldemort and his supporters... it's sooo sad....

He died in the fifth book, the Order of the Phoenix. He fell backwards through a veil while dueling with Bellatrix and the other Death Eaters. And may I just say that is so incredibly lame. This character, who barely had to study for his OWLs, who was funny, and brave, and looked out for Harry, fell backward through a veil and is never seen again. He might as well have drowned in an inch deep puddle of mud.

Bellatrix and Sirrus were dueling in the Department of Mysteries, and she hit him in the chest with a spell. His back arched and he fell into the mysterious ragged veil in an arch thing.
I'm not sure if he is dead.
