I have looked at this cartoon several times and I can't understand
what the caption has to do with the cartoon

History doesn't repeat itself fits pretty well. The League of Nations, formed after WWI, did die from lack of exercise. It totally failed to react when Hitler started gobbling up pieces of Europe, trying to use diplomacy and appeasement instead of military force to stop Germany. WWII was the result.

The figures shown approaching the grave are Harry Truman, US President, and an allegory of the United Nations, armed with a sub-machine gun. This is obviously in reference to the UN's response to the North Korean invasion of South Korea. Truman responded with US military support and the UN condemned the action. Then UN member states as small as Luxembourg and as removed as Colombia sent combat forces to Korea to defeat the North.

So history is not repeating itself. Instead of rolling over and dieing like the League of Nations, the UN is actively combating aggression.
