Well the short answer is when the baby has grown enough the hormones in the lady's body send a signal to the muscles in her womb (which is the bit the baby grows in) and the womb starts to get small so it pushes the baby out though the vagina which is the way the sperm got in in the first place. It's a bit more complex than that but that's the basic answer.
