Unless they have reason to suspect that you're not telling the truth about drug or alcohol use, they have no reason to do a drug screening. They also have to tell you what tests they're running if you ask. Something to think about though is if you're using drugs or alcohol to the extent that you're worried about testing, you really need to be honest with your doctor because that could be causing you problems. Not preaching at you, just want you to think about your health.

A thing to remember is that if they run test without your permission you have legal rights.

I would suspect if they noticed you had high liver enzymes they are worried for other reasons. Your body has a mechanism for dealing with organs that are not in homeostasis. It is called negative feedback. How it works is by bringing your bodies system(s) back to normal functioning level.

If I were you I would be concerned about it just as much as the doctor is. Liver problems are not fun for the people who suffer from them. I would be honest with the doctor if you have put yourself at risk for contracting hepatitis by blood to blood contact (sharing needles, unprotected sex). So it's really up to you to decide if you are putting yourself at risk. If you have contracted a disease it's better to know so that you can get the correct medical help so that you don't suffer later.

I say this with concern for your health and for you as a person.

They can tell a lot of things when they do blood tests after you have a test that comes back with high liver enzymes.

Why are you worried? Are you using drugs and alcohol?

Alcohol can cause that. Tylenol over dose can cause it. Being fat can cause it. OR, and what they will want to test you for, is Hepatitis and possibly biopsy your liver to make sure you don't have cancer.
