They will basically take your height and weight. But at some physicals, they may ask you to undress and make sure everything is as it should be.
They just want to make sure you're healthy and capable of playing as an athlete.

Sports physicals are pretty basic. Listen to your heart and lungs, check your blood pressure, urine sample, hernia check, and they get a basic family medical history.

they have to stick their finger in your butt so make sure you poop before you go to the appointment... and they squeeze your balls and tell you to sneeze or they won't let go... then they pinch your nipples to check for cancer... it's very complicated stuff but it's for your own good.
