A lot of people who have delusions don't realize that they're having them. If you see or hear things that other people don't, that's a sign. If you think things that other people find unreasonable -- like voices telling you to do bad things, or that aliens are aiming radio beams into your head through your fillings -- those would be signs that you're developing schizophrenia or a mental illness. A good doctor can help you sort out what's reasonable or not reasonable.

People who have delusions from primary mental illness, don't realize it when they're having them. They actually believe the delusions, that's why delusions are false belief systems. It's thought to be a defense mechanism, but whatever the mechanism, it's a sign of psychosis. Some delusions are not signs of psychiatric illness and are temporary. These may be caused by metabolic factors on the brain like adverse drug reactions or withdrawal.

You have to recognize it in the earlier stages. If it doesn't sound logically possible, it's a good bet there's something wrong.

Voices that you recognize are not your "thought" voice and not anyone physically around you talking about things they could not know and thoughts of extreme paranoia or conspiracy theories are just 2 symptoms.

There's probably a chemical imbalance that needs to be corrected
