I dont know of any places where there are examples of this but it is similar to a behavior plan like what we use in school ssometimes. The key to success is to find incentives and consequences that matter to the student. Then the other key is to make them happen at short enough intervals for the incentive to seem attainable to the student. For example let's suppose we have a student who likes to eat at a certain restaurant. A good goal would be to check with teachers each Friday to see if 3/4 of the homework for that week was completed. You dont want to start off with saying 100% because that will seem very difficult to someone who is accustomed to doing zero%. So anyway if the student attained the goal, reward by going out to favorite restaurant. Chances are that simply by getting more HW done the grades will improve. Test scores may as well. WHat you cant do it say, "If you get straight A's at the end of the school year we'll give you this wonderful prize." The goal, however wonderful, is too far away and looks waaayy to hard to achieve. Take baby steps. Work on attendence and homework completion. Then start looking at improving work quality and grades. Hope this helps.

A agreement is rather anything the photographer will have to draw up for you. If no longer I might no less than have anything in writing that claims how a lot and whilst repayments are due. Plus furnish them with a record of folks and in what mixtures you desire photographed
