Several years ago, Muscular Dystrophy used a poster child in their telethons to raise money for the cure of the disease. Sadly, many use that phrase to make fun of someone else. It basically means that your either deformed in some way, handicapped in someway but yet you are still able to walk around. This is as close as I can get to the reason.

It is a picture of a "child" in the case you cited who is ALIVE and who is representing some cause.. i.e. leukemia.. This would mean that the child pictured... is alive.. and has the disease leukemia .... and is perhaps making speeches or at least showing up for leukemia events(if it is a child, the parent(s) would be along also of course)

A poster child is supposed to be perfect. A child that is ''a gift from God'. No flaws, no hidden agendas, whatever. They do everything they're told, when they're told, never talk back. They're good at everything they do, they want to do things that better themselves..............basically we're looking at a modern day Jesus.
