Plan out your studies with 'to do' lists of the subject matter. You need a place without distractions; no people, music or TV. Put your phone on Silent.
Obviously, you might think you need these things, but you don't. The problem is that the habits might be set. Use a timer and mark a short block of time, perhaps 30 minutes, and keep away from your distractions. Then take a break and play for 10 minutes. Then mark another 30 minute block. Record what you did and for how long. Try increasing the length of the block of study time.
Try and go to bed and get up every day at the same time. You would be surprised at how much you get done if you set new patterns of habit.
The last thing, do not even think of looking at the material the night before the exam. Get a good night's rest and get up at the same time in the morning.
This is coming from the world's biggest procrastinator and slacker. But, I got through it. If I can, you can.
Good luck!

If you have plenty of time before finals, write out a study guide that you yourself make for each of your classes. When you write things down, you learn them much faster and better. And then, when it's time to study whether it is the night before or a couple days before, you can read and re-read your study guide. It may help you to go to the library where you wont be distracted.

I summarize all the materials used for the exam, and re-read it aloud. I think this method is the best for me. It is time consuming with all the re-writing and stuff, but you can remember the stuff better.
