I have always understood it to mean that the person is being spiteful, they are cutting their nose off even though their face now looks horrible and im sure it was painfull just to prove a point or to have final say in something. For example... we go out to dinner im starving but im so mad at my friend for not letting me pick where we go that I just sit there and refuse to eat. Its being stubborn and going to great lengths to prove a point or get even with someone. Hope that helps.

Meaning: Don't do harm to yourself to disadvantage your adversary.

It isn't like it will hurt them if you cut your nose off. Right?
