Mine was 126 pages in Word with 12-pt. Times New Roman font and 216 pages in a 6" x 9" paperback, so about 58.3 Word pages per page in the actual book.

It depends on your page size and the font of your text. The smaller your page size, or the bigger your text, the more pages you'll have, and vice versa.
For example: 820 words on 2 letter size pages with size 12 Times New Roman becomes 5 pages when you change the margins and give the document a multiple pages book fold. This option is under Page Setup on the first tab.

You usually go by word count, silly--if you want to form a "novel". :0)
(Take the number of words you have and divide it by 250)
There is no set 'standard' in MS Word or Works which will give you a pre-assigned book template.

Depends how long your book is. Maybe 45 pages in size 12.

Depends on how they do the printing and/or typesetting. For paperback I'd say it's probably close to a 1 to 1 ratio, considering how close they print the type.
