File an affidavit in the court of the civil judge stating that you want to change your name. It is a simple procedure which will be over in one sitting.

You should probably call someone at your county courthouse assuming I am talking to an American citizen. You'll have to go before a civil judge with a request for a name change. I'm not sure of the exact procedures. I'm also sure each state does have little differences too.

P.S. Rams is correct. It is an affidavit. I didn't remember that.

you only have to go to your courthouse and seek the proper procedures. i know some people that have changed names
