it isn't really her (Cameron Diaz).... it's all in his head. after his accident he signs up for this program where he can "live in his head" and have the life he wants. He fantasizes that he;s with Penelope Cruz, but in real life her never really got together with her. In real life he and the blonde woman are "f^(k buddies".... In the scene you're asking about, he's in a sort of virtual reality program, but there's a glitch that switches women on him... he's so freaked out when she changes faces in the middle of the deed that he strangles her. Nothing really happens to her, though, because it's all in his imagination....

I don't recall that movie very well but some people like to be choked while having sex because it cuts off oxygyn to the brain and makes for a more intense experience. That could be what they were doing in the movie. I have never heard of smothering as a way to achieve this but hey ya never know with some people.
