Take a piece of tightly woven cloth twice the size you want the pillow to be. Fold it in half & sew up 2 + 3/4 of the open edges. Turn it inside out and fill it with buckwheat (or rice, for that matter) but not TOO full, so it'll conform to the bodypart you wanna warm up. You can also add some aromatic herbs at this point. Sew the open section closed and you're done!

You're kidding right? Sew up a cloth bag filled with buckwheat, put it in the microwave to get it warm. Place it where you want the heat. You can use other seeds or grains.

Um, my poor mum would take a towel, pop it in the oven and take it out and just use it, wait till it wasn't hot again and put it in the oven again.
you could also do this with a dryer.
