Like everyone said, it varies depending on where you live. I usually spend in the neighborhood of $300-400. If you go too cheap, generally the pictures aren't going to be that good. More expensive doesn't necessarily mean better, either. The really expensive photographers are often just hyped.
Color photos are the thing now, so you'd be wasting your money to get black and white shots, especially if you're interested in doing on camera work or getting an agent.
Find a couple of headshot photographers - and they MUST specialize in headshots, or the pictures will just not work - and compare their websites or portfolios. Any good photographer will have a portfolio of some kind for you to look at. If they don't, don't use them.
Get recommendations from actors/agents/etc in your area. Word of mouth is the best resource.
Meet personally with the photographer. No matter how good he is, if you aren't comfortable around him, the pictures won't work.
Your friends, relatives, or even a photographer who specializes in weddings can't do these pictures for you - people who get their photos that way are glaringly obvious and come off looking unprofessional. That doesn't mean you have to spend $1000 (I'd NEVER spend that much on pictures that I replace every two years!), but it does mean you have to do your homework and find the right person.

It depends on the city in which you live. In New York City a headshot can cost as little as $100. I heard of a headshot photographer in Connecticut that charges $1,000.00. The most I paid was $800.

Varies greatly. I'd say the most important thing for a newbie to remember is to use a HEADSHOT photographer. Not a friend, not any other kind of photographer, a headshot photographer.

It varies widely from city to city. One thing I would advise, though, is make sure your photographer releases the rights so you can get them reproduced inexpensively.
If the "shoot" is $100 or less, that's pretty cheap. There are companies that can reproduce them very inexpensively, and include your name, phone number, etc. I have used this company and thought they did a great job. BTW, my photographer was totally cool with my using them, and even gave me the info! Some can be really difficult, though, and insist that you get prints from them which is where it gets really expensive!
