Try a pumice stone when all else fails.

I have hard water in my house and tried many chemicals to clean my toilet and found the best solution was a Pumice stone I got from a hardware store. You just have to shut off the supply valve, flush toilet and use the stone a little at a time (with the little water left in the bowl) until you get all the scale and rust off..it works! You just have to be patient and do a small area at a time. You may choose to use rubber gloves also! Pumice will not hurt ceramic or porcelin (toilet/sinks). I had to use one stone to clean my toilet...yours may take more/less depending on the build up.

I use the works toilet bowl cleaner you can find it at the dollar store pretty cheap. Squirt it in the toilet and let it sit for a few minutes then scrub repeat if necessary. But a word of advice works toilet bowl cleaner contains acid so don't let it sit too long especially if the toilet has any age on it or it will eat the glaze off the ceramic. I know it went from a $3.00 toilet cleaning to a $100.00 toilet replacement in a really short time

CLR or Lime-away. both are effective.. they don't work instantaneously, though (like they advertise on TV). Expect to soak and scrub.
