At 4wks preg, I had an ultrasound but it picked up nothing, so I had to have an internal scan where it picked up one sac...nothing inside it, but they honestly said it was too early. I'm going back next week (which will be ten days later) and I'm wondering if I'm likely to see anything then? Also, is it possible that another sac could develop, or there could be two heartbeats in one sac? I only ask, because my HCG levels are extremely high - 13,540 at 4wks.

You're not likely to see a heartbeat until you're about 6 weeks pregnant, so you *should* be good by the time you get to that u/s. If there was more than 1 sac, it could've been hiding, or it could've been in an even earlier stage in development (fraternal twins can be conceived as late as 10 days apart, IIRC). It is also possible, but VERY rare, that there could be 2 babies in 1 sac--those are called monochorionic/monoamniotic twins, meaning they share an amniotic sac & placenta. This is the case in roughly 1% of identical twin pregnancies.

Identically twins would share a sac, and fraternal would have separate sacs. It is possible that you will be able to see something then, at this far along it is really hard to see anything because the baby is really small. Some 7-8 week ultrasounds can not see anything.

Sometimes monozygotic twins (that developed from one egg - identical twins) can indeed share a single amniotic sac and placenta. However, you were so early on when you had that scan, best to wait and see what the next one shows up when hopefully things will be a little clearer.

It would be kind of odd to have an ultrasound at only 4 weeks - nothing would show. I can't think of any justifiable reason to have an ultrasound that early.
if one sac, then either one child or twins that shared the same egg. If two sacs then twins came from two eggs.

it could be something called a molar pregnancy but this is increadably rare.
i think it was just too early to tell / see the baby. i had a scan at 6wks and you could only just see the baby (about 2mm long!) bu 8 wks it was huge!!
theres a possability im carrying twins too but have only seen 1 so far (high hcg levels too and swelling up quite quickly!)
next scan on 22nd so ill no for sure by then

Some twins (identical) develop in the same sac. Others (fraternal) each have their own sac.
It is possible for the 2nd one to hide behind the first in an ultrasound.

@ 7 weeks i had an internal scan - the teck said it's just one i found out monday it's two so anything is possible- it's just to early to tell give them a little more time (don't always believe them this early)-yes you can have two in one - i did but i was 7 weeks
