it could become one if you actually believe one day that that fantasy brother exists. That could be the beginning of a psychotic disorder which include delusions.

it sounds to me that you have an imaginary friend type thing going on. when i was little i pretended that i had a sister that was always in trouble. unless you send your brother Christmas cards and try calling him on the phone i think that you are OK.
