If the girlfriend stays more than 10 times in a one month period..
or longer than a week straight

yes
she is considered a tennent and NOt on the lease.. thus breaking it

Read your lease. If it requires you to list the adult residents and if she spends too much time at your apartment then you could be in violation of your lease.

Landlords cannot restrict occasional overnight guests. Most leases will define what constitutes "temporary."

If your lease doesn't address the issue of temporary guests and does not require all residents to be listed on the lease, then there's nothing that your landlord can do.

A landlord may terminate your lease at anytime for any reason with advance notice, generally 60 days. I take it like most people you never read the lease you signed.

depends on the state you are in. Your lease should mention overnight guests somewhere.
