I was doing a period comedy once and Jessie and I had some VERY quick changes (under 1 min on average) Most of our costumes were long and easy to undo and yes you guessed it Jess stepped on the hem of hers ripping the entire back open and proving we definitely were not in 18th century underwear. I couldn't help it. I tried not to laugh but had to cut my lines short and belted off stage.
About three nights later I lost my voice and with no understudy did two & half hrs spending maybe 10 mins total off stage and trying to speak with a mouth full of every type of throat soother the cast and crew could find. It hurt so much adn you still couldnt hear me past the first two rows.

Luckily, I never really made any mistakes, but I had to live down a couple of other on-stage incidents.
In college, I was in a funky version of "Midsummer Night's Dream". I was a fairy, wearing footless tights, and during the first little dance I sliced open the bottom of my bare foot on a nail that had fallen from a set piece. After leaving bloody footprints all over the stage, I continued the rest of the performance wearing moccasin slippers!
In High School, my mom made me enter a local pageant wearing the typical 80's Scarlet O'Hara off-the-shoulder hoop gown. I am 5' tall and my dress had about the same diameter. It was also quite low-cut. We all had to go up and down these steps and platforms on stage. Apparently, my dress and I became the subject of massive backstage betting as well as some rude comments regarding cadet buttons and some mini-series ("the North and the South"?). Fortunately, the brother of one of my friends defended my honor and bet in my favor. As my dress never slipped, he found himself a few bucks richer.
Don't be discouraged about your recital, everyone has off days.

Last night, I played in a bar a few towns over. I always need a few drinks to calm my nerves onstage, but sometimes it makes me a little careless. I accidentally left my capo on my guitar for a song, and I somehow didn't realize it until I started singing. In case you don't know, a capo is a clamp that rests on the neck of the guitar. It comes on and off easily, and it raises the pitch of the strings half a step on each fret. So with the capo high on the neck on a song that didn't need it, I was singing ridiculously high pitched, mostly out of my average range, too. I could have stopped and taken off the capo and started over, but it was my first time at this venue, and I guess I decided it was better to sing really high than stop and start.
Probably a bad idea in retrospect. :)

The most embarrassing thing that ever happened to me was during an "open dress rehearsal" of HMS Pinafore. I was in the chorus, and at the end of the first number all the sailors fell down into each other's laps. Well, my pants split all the way from back to front, revealing my "tighty whiteys" to the world!
