depends on what you consider wrong... cause theres a lfew things that can...

first the obvious, your main chute and reserve chute fail, Im my opinion, your pretty much FU**ED... so one time for that type of mistake,

now if your talking pulling your chute late, and coming in to hard, you could break a bone, but other than that, you should be ok to jump again.. same goes for landings, trees wires, water etc.... i wouldn't make a habit out of making wrong choices with that activity.

heres a little sidenote:

roughly 30 skydivers die a year in the USA, so thats about 1 for every 100,000 jumps. According to U.S. Parachute Association (USPA) statistics, 1998 was the worst year on record for U.S. skydiving fatalities, with 44. in 1999 – 27, 2000 – 32, 2001 – 35, 2002 – 33, 2003 – 25, 2004 – 23.
It may sound like a lot, BUT when you consider about 40,000 people die each year in traffic accidents. That’s 1.7 deaths per 100 million vehicle miles.

So if you drive 10,000 miles a year, your chance of dying in a car crash is something like 1 in 6,000. You would have to jump 17 times per year for your risk of dying in a skydiving incident to equal your risk of dying in a car wreck if you drive 10,000 miles a year.

jump away!

Twice. First time, your co-jumper kills him/herself by leaving you with the (much smaller) reserve chute. Second time, you have no co-jumper.

The first incident has happened. That's why I ain't gonna jump out of no perfectly good aeroplanes...

Quite a few times. It's forgetting to open the parachute that's the killer.
