While a photo album is a great idea how about changing the idea a bit-instead of just pictures of her family and friends-go to a libruary and get copies of world events of all types-that took place at her birth or near it. Also add other big events that happened over her life span and the photos of folks that came into her life at those times.
Starting out with a pic. of her at birth and now with the time line included.

Always used to make up a hamper for my gran, with all the things in that she really loved, but never used to buy for herself. Things like special marmalade, lovely biscuits, really good chocolates, an individual christmas pud. Put them all in a cardboard box covered with wrapping paper and padded out with tissue paper, cover with clear wrap (or even cling film) add a nice ribbon and you're done. Gran used to say she looked forward to mine every year.

I think that by personal she means something that you have made for her. My nan used to like my poems - I would write it out neatly and decorate the border then get my mum to get it framed.

Alternatively do you know what sort of music she likes or what her hobbies are. Then you could buy her something to do with that.

Try painting her a picture or something like that, my gran used to really love that sort of present.

Well, a hug, smile and a kiss would do!! Making her know you love her is the best way, maybe by what she meant by personal is that something in action, not in material ...
