You should talk to your doctor, they have some custom ones. You might have an odd shape or size of ear canal, an ENT could tell you that. A hearing aide store might be able to make some suggestions too as they specialize in that area.
Other than that you might want to look at Bose headphones...they will block out the noise and cover your entire ear so you can avoid the ear canal issue.....but I bet they ain't cheap. There are ones that cover the whole ear at hardware stores too, might not be a bad option.
Who cares if everyone else wears the ones inside.....if its more comfortable go for the big ones.

Well, there is the big "over the ear" kind of protection devices-- they look like stereo headphones. But, they may seem unsightly and become uncomfortably warm.
Here is something that you may be interested in. It is called:

Get ear Muffs. They look stupid but they cover your ear to reduce the noise rather than sit in your ear canal. The do get hot however so I suppose that is the trade off.

Wear the sound mufflers that fit OVER your ears- nothing goes in the ear canal- I have a pair - use for gun practice -D
