Books have sometimes odd titles that get people inflamed.
One book was banned, "Rape on the Coast", which detailed how big business and the housing industry was destroying the delicate shoreline.
No one read the book beyond the title, thus the banning.

The meaning is that the cover of the book really means nothing. You can bind a worthless book beautifully and a great book could be in a plain cover. At one time publishers used to bind bad books better so they would sell for people who wanted pretty books on the shelves and didnt care about content. It has become a connotation for judging a person by their looks. All ourward appearances do not tell you what a person is like on the inside.

Sorry I don't know the origin, probably ladies sitting around having tea a long time ago, but it means that you shouldn't judge people based on appearances, shouldn't judge anything based on appearances. You don't know how good a book is until you read it, and you don't know how anyone or anything is like until you get to know through experience.
