Do you mean in the movies or in the comic books? In the movies, not yet, but in comic books, yes. I once read this issue of the FANTASTIC FOUR where he was shown looking at himself in the mirror while in solliloquy. He describes himself as hideous-looking and how much he hates Reed Richards for making him "heavily disfigured". At the end of the story, he was shown as this very good-looking guy with a slight scar on his right cheek (a result of a lab accident with Mr. Richards).
Victor von Doom is so vain and egoistic that he thinks that single scar makes him imperfect. So he has to hide himself behind an iron mask.
However, there's a later story that came out in the mid-80s courtesy of Mr. John Byrne which showed him heavily scarred as a result of the very painful process of putting the molten-hot iron mask DIRECTLY on von Doom's face. (This drove him mad.)
This latter story is a revisionist one. I think the story made by the original team of Stan Lee and Jack Kirby is much better and explains exactly Dr Doom's superiority complex and infallible wisdom (to his mind).

After the accident, only Sue Richards has seen it.
modify) alien life form is not completely correct: that is the origin as first told by Lee & Kirby, way back when.
additional modify) Actually, Lee & Kirby saw his face somewhere in the 1st year of the original FF; Doom visited the Marvel Comics office & forced them to lure the FF into a trap.

Yes and no. They have shown it only partially, around the eyes, showing how hideously scarred it is, or around the edges of the mask when he takes it off to clean his armor oslt. But, durig Secret Wars I, when Doom temporarily stole the power of the Beyonder (after getting hald-dissected in the process), of course he fixed his face. Presumably, he retained his original appearance, so Reed Richards would know who he was when he gloated, but still, he was one handsome guy. I guess the idea is that he is now as inhuman and ugly as he once was handsome.

I believe that they have. He is "horribly disfigured", I think, much more than a simple scar on his cheek.
It was in a laboratory accident with Reed Richards - good call - but it was more like a chemical burn or a fire or something drastic. That is why von Doom hates the FF so much.

In the movie, they show the guy's face before he's turned into Dr. Doom, but I don't know about after that.

they've shown it a number of times before it was scarred... but since then they've never actually shown a full view of his face. most of the time it is in shadows or bandaged.

i agree with alien life form's answer. i heard the same information but i have not actually seen the books which show this happening. i would include the references but at this time it would take a great deal of research.
