Should it be from my son, or from the family?

If your son wants to, and is OK with handing the teacher a gift, it would be nice to let him be the giver. Go with him to the store, pick a few appropriate gifts out, and let him choose which he would like to give her. Put a tag on it that says, from Billy to Mrs. Teacher's name, in his handwriting, maybe he can help wrap it, even if it is just handing you pieces of tape.
Send a separate card from you, wishing her a merry Christmas, and tell her in the card that he picked the present, helped wrap it, and how proud you were of him.

I am a teacher...I would say have the gift be from your son.

You could send the teacher a Christmas card from the family either by giving it to them personally or by having your son give it to him/her separately. Have him say something like "this is for you from my mom," but make sure he hands him/her the gift at a different time (depending on your son's age, I would say send the gift with him one day and the card another to avoid confusing him!).

From your son, find something together and have the son give it to the teacher. It also shows your son good manners.
