It can but I've had an emergency ultrasound at the ER and my bladder was empty. It made it harder to see the baby but they were able to find the heartbeat and I still got some good pics (but I was only about 15 wks at the time).

If your bladder wasn't full enough to see the uterus then they would have done a transvaginal ultrasound. That is the only ultrasounds that I get for the first few.

YES!!!! A doc doesn't instruct you to drink all of that water if it wasn't cruicial to see things early on. That would be just plain mean!! A full-full-full bladder is crippling!
However, I have a friend that swears you don't need drink it....I beg to differ, though.

yes it can when your bladder is full it makes it easier to see your baby as well as the babys organs and if your far enough along it makes it easier to tell the sex.thats why the doctors tell if you havent drank 48ounces worth of fluid to come back

It actually makes the bladder more easily seen and pushes the uterus in an easier seen position.

yes, my doc told me that it can effect measurements of the brain and other parts of the body they need to get. and while it's evil of them to tell us to do it, as soon as they have their measurements they need with a full bladder, they let you go to the bathroom before they finish the exam.
