Probably just to be honest. I have to mend a very important friendship. I fell out with a friend of 12 years a few years back over what I know realize was total crap on my part. I am afraid but I am going to call her and tell her I am sorry and that I love her and hope she can find it in her heart to forgive me and trust me again.

maintaining and mending. Two different things, but to maintain I recommend always treating your friends like family. I always consider my friends, my family. I am there for them 24/7. I have boundaries with them too, and they know it. Now communication is key, maintaining or mending.
Mending... it all depends on the what, who and why? But once talking honestly and having all issues resolved, making sure that you both stay honest with one another and talking things out rather than holding on to issues and letting them build up. Should help you in the long run.

I wish I knew the answer to that...you just don't give up on them, I guess. Give them space maybe, talk to them, be there for him, let them know you care...but overall, always let them know you're there for them (unless they don't want you to be, then you back off but don't be mean).

accept who she/he really is....always be there not only in good times but especially during his/her worst times.....that's what real friends are for.
