If you can live with it, depending on your age, you should. I have a stage 3 varicocele, which means basically the entire left side of my sac is a varicocele. But it only causes me discomfort after severe cardio exercise, and if you lay down, it will provide relief.
The only thing that a varicocele can cause medically other than discomfort is infertility. If your nuts get too warm because of the varicocele, they'll stop producing sperm. When it's time to have a baby, that might be when to look at having it fixed. Infertility caused by a varicocele is temporary.
However, I hope you have had this checked out by a doctor, because a varicocele could be caused by many things. So your urologist should recommend renal and testicular sonograms to be sure about everything.
Again, if you can live with it, don't bother with the surgery, it's too big a deal for something that has no medical side effects except for maybe a little discomfort. BUT! Be sure you talk to a urologist about it.

A varicocele is caused by a malfunctioning valve in a vein. The vein can't drain, and inflates with blood. If the condition can't be relieved with conservative therapy, there is an operation, similar to the operations for varicose veins or hemorrhoids. Basically, the offending vein is either ligated (cut), or embolized (blocked off). This allows blood to flow through alternate paths, and relieves pressure on the spermatic cord.

Hey there, I've had to vericocele myself a few years back. I'm 27 now, but when I was 20 I had to have an operation. Let me tell you the operation itself wasn't that bad, just an hour, but the recovery was miserable. Much worse than what I had beeen prepared for by the doctors. Doctors said I would be back to normal activity in three weeks, but at three weeks I was still feeble and barely able to stand up for more than 5 minutes. It took almost a whole year for me to really feel back to normal and able to engage in normal activity. But, still to this day, I cannot lift heavy objects or exert myself too strenuously b/c it will only bring on a relapse of the vericocele.
Some things the doctor DID NOT tell me prior to this ELECTIVE surgery 1) your penis will get smaller b/c of the surgery. I lost about an inch in length and seem to be losing a bit continuously. 2) Recovery takes an incredibly long time. Your muscles will essentially atrophy while you're recovering and that, coupled with your inability to exert yourself, will leave you extremely weakened. 3) Surgery to correct vericocele on one side, does not completely eliminate the possibility of it coming back to that same side, and spreading to the other side. At the moment, I have the vericocele coming back and spreading a bit to the other side of the abdomen. 4) You won't be able to stand up for too long even after surgery.
I had a very bad case of vericocele and so I pretty much had to get things taken care of. But, if it's not that bad I would recommend putting it off as long as possible. Instead, I would just make lifestyle changes to that might discourage the vericocele from getting worse- not lifting weghts, standing for too long, or being too sedentary.
Good luck. Vericocele sucks, but it's not the end of the world.
