If you don't use a towel to wrap it in and leave it on the skin for too long causing FROSTBITE it can.
Cold or heat should only be applied for 20 minutes at a time with 20 mins off in that cycle of intervals for the first 24 hours. After that only 20 mins at a time.
BTW: It's worse with kids than with adults because they have smaller bodies that can't stay as warm as long as an adult, but adults can get these type of injuries from heat/cold packs, too. It doesn't matter what KIND of pack it is!

Yes, it can cause injury. A lineman for the Pittsburgh Steelers football team, just a few weeks ago, put a cold pack on an injured ankle. He left it there too long. He burned the skin bad enough that he had to miss two games to allow the skin to heal.

I would expect so. Either extreme will kill skin cells. I have small gel packs and I put each of them into a sock and leave it there. When we take one out of the freezer it's ready to apply. Ice should be applied for 20 minutes on an adult. I would only use it for 5 minutes at a time on a child. Once/hour.

Don't put a cold pack or ice directly onto their skin. Place a towel between it. Like a hand towel.

yes......just think of dry ice. It can burn a small childs skin because they are much more sensitive to heat and cold.

no it cant because its cold but if it is too cold it could cause frost bite but you should always read the safety precautions on anything that concerns a child if that doesn't work just unplug it
