I want to know the answer to that too.

Because they contradict themselves on that show. Sometimes people can hear, or at least understand him, but then they sometimes only show conversations between Stewie and Brian.

It really does depend what the situation that Stewie is in. Brian is the only character that can always understand him, while other characters, both reoccurring and one offs can hear him. Seth MacFarlane (the creator of the series) has said the following on the topic:
MacFarlane has asserted that he and his staff will never take a definite stand on whether Stewie can converse with adults. Whether the other characters understand him or not is left to the writers, who can decide which possibility is most appropriate for a given situation.
Stewie with Brian, the only family member who takes him seriously.
Brian is the only character whose ability to understand Stewie is unambiguous; the two generally converse "normally." In one instance, they are apparently able to hear each other's thoughts, in Running Mates.
Despite this, in the DVD audio commentary for "E. Peterbus Unum", MacFarlane states that adults can understand Stewie, but don't take him seriously, "sort of like... if a 4-year-old who [could] talk told you to **** off." This concept is supported by the other characters often looking at him as he speaks to them, as if to acknowledge him, although paying no mind to his often-ungracious addresses, such as calling Lois by her first name (although a few times over the course of the series, he has referred to her as "mother"), referring to Peter as "the fat man", Brian as "dog" or on occasion "Dogbert", but most of the time simply calls him Brian, and Chris and Meg as simply "you there". The other characters also seem to have blocked out Stewie's odd behavior, though Stewie may be speaking in 'baby' and his lines only come from what he thinks he is saying, and Brian can understand him because he is the only intellectual in the family.
Non-recurring adults often clearly understand the content of Stewie's speech. In Stewie Griffin: The Untold Story the other characters, including "Stu", can understand him. Though this may simply be true because "Stu" is Stewie's future self.
At the end of the episode "E. Peterbus Unum", there is a self-referential joke relating to this issue. The scene features a class in the distant future watching a "historical" video of events from the episode presented to them and a student asking, "So... can the family understand the baby or what's the deal with that?"

No, I used to work as a producer for Family Guy. It seems Brian is the only person that can hear him. Peter, Louis and the rest of the family can only hear him as a baby talks. For example, Stewie curses at Loius " Give me a cookie, vile woman!" But Louis hears him as he's saying " Can I have cookie Mommy?". We only decided to play Stewie as an evil child in order to give alittle humor for the baby. An inoccent baby would be alittle boring, wouldn't it? ( Maggie from The Simpsons took that role, ha ha)

Brian can understand Stewie. The rest of the world only hears baby noises. It can be confusing, but after you watch a while, it becomes clear.

This answer is gonna suck, but it's the right one: The writiers of the show want you to ask that question. It's an ongoing joke and they have even mentioned it a time or two in a couple of episodes. All indications are yes, but you can't be 100% sure, and that's the way the writers want it. Just like you never saw Steinbrenner's face in "Seinfeld" or never saw "Vera" in "Cheers."

He is only one so everyone would only understand what a oen year old could speak. Brian is the only one that can completely communicate with him. I am pretty sure that Brian is actually human but looks like a dog. ;-)

Brian and other toddlers can always hear him. The rest of the family can sometimes understand him and sometimes not.

Brian can. None of the other family members; other main members can't. Seems like the strangers can though.

Yea..I used to ask the same thing..and they can! I watched about 100 shows in a row to find out..after the mom got on to him for saying a bad word thats when i knew

only brian can understand him as an adult he talks...but his other family members thinks...when he speaks...he's talking like a baby...if that makes sense?
