The isotope is bombarded with neutrons. upon fusion twice as many neutrons are created and bombard more isotopes etc. Look up fusion on the net for a more exact diagram.

The pressure is enough to keep all those hydrogen atoms very close to each other and the temperature is high enough to keep them smashing violently into each other. When one hits another with enough force, it merges with it to make a new, heavier atom, which releases energy.
