There is a charlie in the box, a train with square wheels, etc. But what is the problem with the dolly?!

The ELF is the one that wanted to be the dentist, not the doll.
The doll has two eyes, also.
There's not anything physically wrong with her. There probably just better dolls out there.

Instead of crying and doing normal doll things she says "How do you do?". It's in the song the misfits sing on the island.

I also wondered about the elephant - who cares if a stuffed elephant has pink spots?
Arthur Rankin, who created the program with Jules Bass and writer Romeo Muller, answered the question (the "number one question we get asked" he said) in an interview once by suggesting the doll's problem might be "more psychological than physical." She is crying when we see her, so maybe that's it.
