I'm not exactly a relaxed traveller but I am 100 times better than I was 2 years ago. It sounds terrifying, but becoming a frequent flier will put you at ease as you will get so used to the procedures, sounds, and everything. Try requesting a seat on or in front of the wing, as I find its usually quieter and a bit smoother a ride if there is turbulence. Always bring more than enough to do- books, magazines, crosswords, music- to distract yourself.
Find out what makes you more relaxed: would you feel claustrophobic without being near a window or will a fear of heights get in the way? Are you more at ease flying at night or during the day?

I have heard of courses in fear of flying, like a group therapy. Or maybe one on one therapy.
Or get: Overcome the Fear of Flying by Glenn Harrold (Audio CD - Jun 21, 2002) - Audiobook, or something similar.

My mother swore she was saving lifes by not going on a plane that is how deep her fear was. Tell a doctor about your fear and book a flight. He will give you some medication to help to keep you calm. Once you have flown it will become as easy as it is to get into a car :-)
