It sounds like you've got a spyware infection that's still hanging out on your system. Even after a repair install and cleanup, there must still be some files or components on the system which are enabling the spyware to load and install itself repeatedly, causing reinfection and therefore causing your internet connection to fail (some spyware cause this, I've had some myself that do something similar).
To remedy this, you may need to obtain a boot diskette or CD which will allow you to run a virus and spyware scan and removal. If you have Norton Antivirus, I think the install CD is actually bootable and might let you run a scan on the hard drive if you put that disc into the drive and boot from it. Failing that, you could look up the Ultimate Boot CD for Windows here:

In fact, this may be your best bet; it includes some utilities like Spybot and AdAware which you can run.
Download the boot cd and follow the directions for installing it and getting it written to a bootable CD (you may need to borrow someone else's computer to do this if yours is completely inoperable right now). Then, use the utilities to scan for and remove the viruses / spyware that are still lingering.

Bad cable perhaps? If one of the 8 wires are broken in a network cable it will cause this reaction.
Are you using DSL or Cable? For DSL you need to set up your login and password (unless your using a router to connect multiple PCs, then the pw and login will be set up there). If you are using a router remove that from the connections (this is NOT your modem) and directly connect your pc to the modem instead. If it works your problem is the router (either settings or hardware).
You did a good thing by trying to switch the card. That could hav ebeen the issue but you know now that it's not. You know it's not software or drivers either.
Try the two suggestions above. You may have to contact your ISP for further info or help if it's the modem. Did you try a power cycle? If not try turning everything off. Power the modem on, wait a min. Power on a router (if you have one in the chain, if not skip this) and wait untill it's active. Lastly power on the pc.

oh one other thing. go start> run type cmd. try typing the following
ipconfig /release (wait a moment after this then type the next part)
ipconfig /renew (the renewal process can take some time wait for another prompt to come up after)

Your computer can not acquire the IP address from your router or from ISP. You need to tell us what's your network setup, how you are connecting to the internet, and then we'll tell you how to fix it. :)

Ok go to control panel > system > second tab > device manager > click on your modem or ethernet card > it should be like the second tab, change one of the setting to 10 half duplex or something similiar this should help.
