You should sit down with the kids, and have them draw some pictures. Have them put them together into a book , using construction paper and some sort of binding. Include a couple of photos of the kids. Help the kids with big words, like I Love You, Mom. ....
4 year olds son't buy things. They make them.

You have so many great ideas here. You have the right idea about including the kids in the gift giving process. If it were me, I'd give them some of these choices and let them vote. Anyone of these would be a hit with mom...I'd go with the whack a mole and the squirt gun also and put it all together as a gift for her...She'll be thrilled!!
PS...one of my most favorite gifts ever was a pretty little smooth blue rock that my daughter found. To this very day, I carry that rock with me in my purse as a good luck charm of sorts. I wouldn' t dream of tossing it out.

What about having a friend or family member take a photo of the 3 of you and frame it to give her? Is it something that a child can give a Mother. They would get excited at getting "dressed up for pictures for Mommy",I am sure

A sterling silver charm bracelet
You can add charms to it that signify special hobbies, events or people in your wife's life.
I have a charm bracelet and my children each pick a charm out for me each year to add to it. My bracelet shows my passions for gardening (watering can), scrapbooking (tiny scissors), Bible study (tiny whale that opens his mouth to show Jonah inside), antiques (old typewriter, telephone, model T and straw bonnet) and cooking (tiny measuring spoons). Your twins can add on each year for birthday & Christmas & Mother's Day and everytime your wife looks at her bracelet will remind her of a cherished time or person.

Ok, Whack-a-mole sounds hilarious, but she' d probably most like something they make that she could keep for forever-a painting, drawing, etc. If you dissolve as much Epsom's salts as possible in water , you can paint a snow scene on dark construction paper and let it dry. In a day or two it will leave the salts on the page and look snowy. Some cute trick like that would be appreciated by her, I'm sure.

Any store bought gift that is practical, she'll know that it's from you. Go to a craft store and buy one of the hand print kits, and put a poem in it. That would be an awesome gift. I received gifts that my girls picked out on their own for several years, and you know what kids pick out. You could take the twins shopping, and say mom wants a sweater, or whatever she wants, and only give them two choices of colors. That way they can say they picked her gifts out.

How about they make their own little coupon books for things like; I will help mom with dinner, or the laundry, or I will run mom a bubble bath.
These are all things she will love to have, especially from her children, and they will have fun making them.

You could get creative with your kids and do a photo collage of them, and let them put their hand prints on it. Or if you have a build a bear close by you could take them to build a bear and they can each make her a bear and you can add their birthstones on a necklace and their name to the shirt. I bet she would like those ideas. I have a four year old and he got me a photo album of pictures he took all by himself. He did a great job.

Let them give a 'big kiss' to her...definitely she will love it! My children when they were of that age used to make their own greeting card (with some paintings) and give it.....my wife still preserves them!!

You don't say if the four year olds are girls, boys or one of each...
But I bet it would mean the world to mom, to have a day at a spa, for her and her best gal pal...to just luxuriate and not have to be a mommy for a few hours!
