I have been living overseas for about 4 years, I returned back to the States 4 months ago and have been at my current job now for almost 3 months, so became out of touch with current salaries. Basically I didn't do enough research on what my pay rate for my skills should be and realize (stupid I know) I requested a salary that is much lower than the local average, about 6,000 a year lower. I am approaching my 3 month end of probation meeting, how should I bring this up? I really need a raise.I am pretty sure my boss will confirm my employment, but I need to mention this. How should I go about doing it?

Be up front and honest. If you don't like their answer, go find another job.

I feel that you should drop some hints to your boss and see what is his reaction. If he is a good boss, I believe he will adjust your salary based on your performance. If your boss is going to pretend he don't know anything, I suggest you should start looking for other jobs that pay better than your current job
