I hate cheaters. What I hate more is people who use mental illness as an excuse. What your friend is doing is making a consious choice. She knows she is married, there is no confusion there. She knows that what she is doing is wrong. There is no confusion there. She hides it from her huband. No confusion there.
I say out her and then tell her you were confused, plead temporary insanity. Her husband deserves to know. This is wrong and she knows it, otherwise she wouldn't be hiding it from him. Do it with both of them in the same room with you at the same time. Don't back down until he doesn't think but KNOWS you're telling the truth. Chances are they will both end up hating you, that is when you plead temporary insanity!

HE is getting the short end and will continue to do so. Not all but many bipolars have an increased sex drive, and cheating is not uncommon with them. He needs to go to counselling and learn more about it. I have lived with a BP wife for 18 years and it is not easy. I almost wish she would cheat on me so I would have a reason to leave her.

This is a no wim situation. If you tell him, then they'll both end up not talking with you anymore. If you don't say anything, and he finds out then he'll ask you why you never said anything. Your best bet is just to keep silent about it. Let him find out himself. If he asks if you knew anything then deny it.
