Can someone HELP with this email form?

There's a couple of things that you need to do. First, check to see if your hosting server has any kind of email service. Most do and it's usually called formmail. If they don't you can usually install one on your own. Formmail is a free script:

Once it's downloaded you need to follow the instructions on the site to install it on your server. It's usually just a simple ftp transfer.

Once formmail is installed, or if your server has a mail script already, you need to put the url of the script in the "action" field. For example:
