Very interesting question... I suppose it can be sometimes. I hate a girl who I used to go to school with because she beat my friends with tennis rackets if they didn't do what she told them to. I suppose maybe she hurt me by hurting them...
I guess you could say almost all the time, hate is indeed a reflection of hurt, but there are rare occasions when this does not apply.

I dont know if its a reflection of hurt. For the most part, i know when you hate is when you are truly unhappy. I mean hate is an empty emotion. You must ask yourself, is the hate worth it? I mean you invest so much into hating something, that in fact you truly lose who you are. It is scientifically proven that hateful words effect the metabolics of your body. I mean your body is almost 90% water, and you retain all of what you say. So, when you become hateful. Is when you effect everything in your life. So yes it is a reflection of hurt, your hurting yourself.

Most of the time, yes.
How ever, hate can be a reflection of brainwashing too. I'm sure you've heard about people hating others just because they aren't the same as them. These others didn't do anything directly against them, they are just different. We call that racism. It's a form of ignorant hate.

It could very well be. Anger is depression turned around so it makes sense that hate is a reflection of hurt.

Hate is a reflection of fear.

If reflection means mirrored-image, then hate's mirrored-image is love.
Edit: Hate is the amplification of hurt, fear and ignorance.

Sometimes. You can come to feel hate towards people that hurt you.
Hate can also exist towards people that you have never met such as hating an entire race of people (or people of other religions, lifestyles, cultures, etc.)

Hate is often a reflection of ignorance and judgement, the way I see it.
Of course, it's different in various situations.
