All the places have been mapped via satellites, but there are several 1000 sq miles that have never been surveyed using modern survey equipment. Mainly in the remote areas of the Rocky Mountains and parts of Alaska. Still to this day there are areas in the Rockies no one has set foot. hard to believe but true.
People have seen the areas through flying and such but never really got out and walked through it.

No, And Area 51 most likely has been mapped by the government, and workers there know the area so yes, it has been geographically surveyed too.

With land based equipment such as surveyors out with instuments, I would say that yes there are probably a couple that have not been detailed. However, it has all been mapped by satellite or by shuttle mission.

No. It is all visible to satellites, and has been for decades.
LandSat mapping was completed long ago.

well, volcanoes create new land every day, though most of it is under the oceans, but new Islands form because of this, so most likely yes, but only small islands.
