I have a headache that makes me wish I was dead, and my stomach is turning flipflops so much that I'm running to the toilet every few minutes. Naturally, I took off of school, but here's the catch: my choir director scheduled a mandatory concert after school, and every after school activity requires participants to be there for 60% of the school day. This mandatory concert will make me or fail me for my 9-weeks grade. What should I do?

Just go. If you throw up, it won't be your fault.

At our school, we have an orchestra concert too.
If we don't go, even if we're sick we have to play the songs for a final exam grade. I haven't been sick yet, but I've always told myself that if I'm sick, I should still go. At least we'll get credit for going.

Call the choir director immediately.
Ask for his/her advice. Circumstances being what they are, you might be given another chance to make up this grade rather than them take the chance on you ruining the concert by puking your guts out on stage. They also will have to consider whether or not they want to out everyone else at risk of catching what ever it is that you have. You will probably have to have a doctor's excuse though.
If you can get to the doctor today, they can prescribe a medication (usually phenegren) that will help stop the nausea. That, and something for your headache, may even get you through the evening's performance. (If they would let you come.)
They cannot force you to go while you are legitimately ill (especially if you are contagious). They do have to offer some sort of make up for this just like any other class. Just expect it to be tough. Make up exams and such are usually designed to make sure you never miss another one.
How old are you?
If you are in college...you have to take care of this yourself.
If you are in a high school or lower, this is a job for parents. Let them battle it for you.
Meanwhile, stay in bed and keep hydrated.

Go to the doctor and get a medical excuse. They can't fail you if you have an excuse from your doctor. It doesn't matter how mandatory the activity is, if the doctor says no, they can not penalize you for it. Then you could work with your choir director about an alternative assignment. Perhaps you could sing a solo for the rest of the choir.
