Assuming that it is actually scanning outbound mail, your machine has probably been hijacked by a bot-net. Your machine is being used to spew spam all over the internet. It appears that it is sending mail through a "server" in Poland somewhere, probably another hijacked machine.

If your antivirus or antispyware software has not found it, my guess is you've been infected by a root-kit bot that is hiding from the OS. You need professional intervention to get rid of that. The best (and often only) fix is to backup your data and reinstall your OS and all apps.

Try an anti spyware in Safe Mode, and also run a rootkit detector;
You may need to switch off System Restore as back up files will be hiding in there, and it will just restore itself. Go to Start - programs - accessories - system tools - system restore.
Run your anti virus and anti spyware in Safe Mode. To get into Safe Mode repeatedly press the F8 key after the bios screen loads but before Windows loads. You'll know if you made it because the screen will be at 800 x 600 and your pc will be slow.

Do you use Outlook Express, Incredimail, or some other application that utilizes POP3 email? If not, someone may have hacked your PC to use it as a mail relay server and may be sending spam mail through you.
